Breast Reduction Surgeries

The main goals of breast reduction surgeries are:

• Reducing the breast to the desired size
• Giving the breast a cone shape with a rounded base
• Ensuring that the nipple is located in the most ideal place
• Giving upper pole fullness
• Maintaining nipple sensitivity as much as possible
• Preserving the lactation function of the breast as much as possible
• Ensuring a permanent result
• Elimination of asymmetry, if any
• Achieving these processes with minimal scars

It is possible to achieve these goals we have mentioned with the right choice of technique and experience. There are many different techniques in breast reduction surgeries. Each surgeon's experience and techniques may not be the same. I prefer the vertical mammoplasty technique, which leaves little scars. In this technique, there is a single line around the nipple and down the nipple. I usually prepare the tissue that feeds the nipple from the upper part (superior pedicle). One advantage of this method is that it adds extra texture to the upper part and contributes to fullness in the upper part. Another advantage is that if the breast sags in advanced ages, it allows a safe recovery surgery. Another advantage is that it is less exposed to the effects of gravity than other techniques. It is possible to safely apply the vertical mammoplasty technique even on giant breasts. There are cases published by me in the literature on this subject. The most advanced case in which I used the vertical breast reduction technique was our patient whose nipple-shoulder distance was 55 cm and we removed 3500 grams of weight from each breast. This patient has entered the medical literature as the longest and largest breast in which superior pedicle vertical mammoplasty has been successfully performed. Another technique with which beautiful results are obtained is central pedicled mammaplasty. Achieving good results in both superior pedicle and central pedicle mammaplasty is only possible with experience. There is no standard pattern in both. There are drawings and calculations that are made specifically for the patient.

After the operation, the patient stays in the hospital for 1-2 days. Thanks to the long-acting local anesthesia performed during the operation, patients do not feel severe pain after surgery. Our patients come out of the surgery with a special bandage wrapped around the breast and wearing a surgical bra. The healing process takes 10-14 days. In general, at the end of the 2nd week, patients continue their daily life from where they left off. It takes 3-4 weeks for the breast to take its ideal final shape. I have patients who became pregnant and lactated after this surgery. However, no matter what technique is used, inability to lactate is also a possibility, since some of the mammary glands and ducts are removed in breast reduction surgeries.
